· The applicant have to file a report with the nearest police station. A full-fledged FIR/GDR is required if the passport is still valid.
Documents Required For Lost Passport
· Current residence proof (at least 2 of the documents as listed below)
· Annexure ‘F’ (duly verified by 1st Class Judicial Magistrate)
· Original FIR/GDR copy.
DAMAGED PASSPORT
Re-issue of passport can be applied if an applicant’s existing passport is damaged.
Documents Required For Damaged Passport
· Current residence proof (at least 2 of the documents as listed below)
· Annexure ‘F’ (duly verified by 1st Class Judicial Magistrate)
· Original FIR/GDR copy.
Proof of Present Address:
· Water Bill.
· Telephone (BSNL landline or Post-paid mobile bill).
· Electricity bill.
· Adhaar card.
· Bank Passbook with One year statement (Only nationalized banks in India).
· Income Tax Assessment Order.
· Voter ID card.
· Proof of Gas Connection.
· Certificate from Employer of reputed companies on letter head.
· Spouse’s passport copy (First and last page including family details), (provided the applicant’s present address matches the address mentioned in the spouses passport).
· Applicant’s current and valid ration card.
· Parent’s passport copy, in case of minors (First and last page).
